Clinical use of convalescent plasma
2.1 Indications for use
Severely or critically ill patients with COVID-19 who have tested positive for respiratory tract infection;
Patients with COVID-19 who are not severe or critical but are immunosuppressed; or have low CT values in viral nucleic acid testing but with rapid disease progression in the lungs.
Note: In principle, convalescent plasma should not be used in COVID-19 patients whose disease course is more than three weeks. However, in clinical applications, we have found that convalescent plasma therapy is effective for patients whose disease course is more than three weeks and whose respiratory tract samples are continuously positive for viral nucleic acid. It can accelerate viral clearance, increase plasma lymphocytes and natural killer cells, reduce plasma lactate levels, and improve renal function.
2.2 Contraindication
For patients with a history of autoimmune diseases or japan number data selective IgA deficiency, the use of convalescent plasma should be evaluated with caution by physicians.
2.3 Administration schedule: Generally, the dosage for convalescent plasma therapy is 5400 mL per infusion, or ≥ 200 mL per infusion for multiple infusions.

1 Classification and stage
COVID-19 can be divided into early, middle, critical and recovery stages. In the early stage, the disease has two main types: "pulmonary edema" (wet lungs) and "external cold and internal heat".
The middle stage is characterized by "intermittent cold and heat". The critical stage is characterized by "internal block of "epidemic toxin". The recovery stage is characterized by "deficiency of "Qi energy" in the spleen-lung". The disease initially belongs to the pulmonary edema syndrome. Due to the fever, periodic cold and heat treatments are recommended. In the middle stage, cold, damp and heat coexist, which refers to the "cold-heat mixture" in TCM terms. Both cold and heat therapy should be considered. According to TCM theory, heat should be treated with cold medicine. But cold medicine worsens the "Yang" condition and leads to the formation of a cold spleen and stomach, and a cold-heat mixture in the middle of Jiao. Therefore, both cold and heat therapy should be considered at this stage. Since the symptoms of COVID-19 patients are often cold-heat, cold-heat therapy is better than other approaches.
